Psychologist
Requirements: Must have a current and valid license issued by the California Board of Psychology

Psychiatrist
Requirements: Physician must be licensed to practice medicine in the State of California with a current and valid Physician license issued by the Medical Board of California. Must be Board Certified or Board Eligible in Psychiatry.

Registered Radiologic (X-ray) Technologist
Requirements: Certification from the State of California, Department of Health Services in Radiologic Technology and certification from the American Registry of Radiologic Technology (ARRT).

Marriage and Family Therapist
Requirements: Must have a current and valid license as a Marriage and Family
Therapist as issued by the California Board of Behavioral Sciences

Registered Nurse (RN)
Requirements: Current State of California licensure and/or certification and a
current Cardiopulmonary Resuscitation (CPR) certificate.

Ophthalmology
Requirements: Must have a current and valid license to practice medicine, specifically
Ophthalmology, in the State of California. Must also have Drug Enforcement Administration (DEA) certification.

Optometrist
Requirements: Must have a current and valid license to practice Optometry in the State of
California. Also, need to be registered to use Therapeutic Pharmaceutical Agents (TPA's) issued by the State Board of Optometry.

Licensed Clinical Social Worker
Requirements: Must be fully licensed to provide services in the State of California.

Dental Assistant
Requirements: Must have current and valid California Dental Radiation Safety Certificate and a Registered Dental Assistant License from the Department of Consumer Affairs, Dental Board of California.

Licensed Vocational Nurse
Requirements: Must be currently licensed in the State of California by the Board of
Vocational Nursing.

Occupational Therapist
Requirements: Must have a current and valid certificate as an Occupational Therapist from the National Board of Certification in Occupational Therapy and maintain a current State of California license with Department of Consumer Affairs to practice Occupational Therapy.
